Your communication must include:

A clear identification of the copyrighted work allegedly infringed (e.g., the URL where the content is located) or a representative list if multiple works are involved. Identification of the infringing material and details sufficient to locate it. Contact information, including your address, phone number, and, if available, email address. A statement: "I have a good faith belief that the use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law." A statement: "I swear, under penalty of perjury, that the information in the notification is accurate and that I am the copyright owner or authorized to act on behalf of the owner of the exclusive right allegedly infringed."

If your material is removed or disabled and you believe this action was a mistake or misidentification, you may file a counter notification. Misrepresentation of non-infringement may result in liability for damages, including legal fees. If uncertain, consult an attorney before proceeding.

Your communication must include:

A physical or electronic signature of the subscriber. Identification of the removed or disabled material and its prior location. A statement under penalty of perjury: "I have a good faith belief that the material was removed or disabled due to mistake or misidentification." Your name, address, and phone number. A statement: "I consent to the jurisdiction of Federal District Court for the judicial district where my address is located and accept service of process from the person who filed the infringement notification or their agent." A statement: "I swear, under penalty of perjury, that I have a good faith belief that the affected material was removed or disabled due to a mistake or misidentification."
